What are your preferred days of the week and times for tutoring? Although each session is only ~30-45 minutes, please indicate all of the general time windows during which your child would be available for tutoring sessions. Please note that the more options you provide, the faster we will be able to make an introduction.

I agree that my child may participate in virtual tutoring sessions with OurVillage and its volunteer tutors. I acknowledge that OurVillage does not charge any fee and does not guarantee any particular results from its tutoring services. I acknowledge that the session is virtual, and so I remain responsible for the supervision and welfare of my child during the session.
On behalf of myself and the child listed above, I release OurVillage from any and all liability, claims, actions, or causes of action of any kind arising out of or related to any loss, damage, or injury that may be sustained while participating in OurVillage’s tutoring services.
